Part One   Classical Chinese Patent Medicines for Cardiovascular Diseases   Chapter One   Classical Chinese Patent Medicines for Coronary Heart Disease   Coronary heart disease refers to coronary atherosclerotic heart disease,also known as ischemic heart disease.The common clinical coronary heart diseases include angina pectoris and acute myocardial infarction (AMI).There are other clinical types,such as asymptomatic myocardial ischemia and sudden coronary death.   Section One Introduction   1 Etiology   The cause of coronary heart disease is related to abnormal lipid metabolism.The deposition of blood lipids in coronary arteries can cause coronary atherosclerosis, inducing coronary artery stenosis or occlusion.It can also cause atherosclerotic plaque rupture with thrombosis blocking coronary arteries or leading to coronary artery spasm.All of these factors can cause insufficiency or interruption of myocardial blood and oxygen supply.   2 Pathogenesis   The pathophysiology of angina pectoris includes markedly increased myocardial oxygen consumption and insufficient coronary artery blood supply under emotional agitation, the increased physical activities,satiety,and influence of heavy smoking and drinking, causing acute and temporary ischemia and myocardial hypoxia.   When coronary atherosclerotic plaques rupture due to the causes mentioned above,together with atherosclerotic stenosis, platelets accumulate on the surface of the ruptured coronary atherosclerotic plaque and form thrombus,which suddenly blocks the coronary artery lumen and leads to myocardial ischemic necrosis,inducing AMI.   3 Clinical Manifestations   The clinical symptoms of angina pectoris include occasional short-term chest oppression orchest pain,which can be relieved in a few minutes after resting or using nitroglycerin.Patients inthe remission period or with latent angina pectoris usually have no symptoms.   Patients with AMI often manifest severe and long-lasting poststernal pain,often accompanied by palpitations,shortness of breath,panic,pallor,cold sweat,etc.,leading to arrhythmia,shock, heart failure,and even sudden death.   4 Diagnosis   The diagnosis of angina pectoris can be based on the characteristics of the clinical symptoms and ischemic electrocardiogram (ECG;abnormal changes in ST-segment and T-wave).Patients in remission period or latent angina pectoris can be confirmed by the positive result of exercise ECG test or 24h continuous monitoring of dynamic ECG and measuring myocardial injury markers, such as cardiac troponin,creatine kinase (CK) isoenzyme,and myoglobin,if necessary.Coronary angiography,coronary ultrasound examination, myocardial isotope scan,spiral computed tomography (CT) coronary imaging,etc.,can confirm blood vessels with lesions and damaged areas.   5 Treatment   The commonly used chemical drugs for angina pectoris include nitrates such as nitroglycerin and isosorbide dinitrate;calcium antagonists such as nifedipine (Xintongding),amlodipine (Luohuxi),and diltiazem;and β-receptor antagonists such as metoprolol and propranolol.These drugs reduce myocardial oxygen consumption,improve myocardial metabolism,relieve angina pectoris,and reduce infarct size.   In the event of AMI,the following procedures,in addition to generic drugs,should be used immediately in the rescue: oxygen or heart massage,mouth-to-mouth breathing,administration of anticoagulants,and thrombolysis.For patients with myocardial fibrillation,defibrillator and percutaneous coronary intervention (PCI) should be used in the rescue.   Besides drug therapy,some coronary interventions are often used in coronary heart disease treatment;these include coronary artery dilation,coronary artery plaque ablation,coronary artery stenting, and coronary artery bypass graft.   In preventing and treating coronary heart disease and angina pectoris,traditional Chinese medicine (TCM) and pharmacy act on multiple targets and can be used in different stages.It can both treat the tip in acute conditions and the root in chronic conditions.However,classical Chinese patent medicines only play an assistive role in AMI rescue.   Section Two Classical Chinese Patent Medicines   1 Blood Quickening and Stasis Transforming   Danshen Tablet/Granules/Injection   (Danshen Tablet/Granules/Injection)   Ingredient:Danshen Root, Danshen   Source:Developed formula,Chinese Pharmacopoeia (2015 edition).   Indications:Quickening blood and transforming stasis.Used for chest impediment caused by static blood blocking and obstruction,manifested as fixed chest pain and dark purple tongue.Used to treat patients with coronary heart disease and angina pectoris with the above symptoms.
